Tier 1 documents require the person with dementia to have legal capacity to sign. As the disease progresses, capacity diminishes. If any Tier 1 items are incomplete, prioritize them above all else. Once capacity is lost, the only option is court-appointed guardianship, which is expensive, slow, and adversarial.
